Complete the Declaration Form
Claimants must accurately and truthfully fill out the Dependants Pension/Widow’s & Children’s Pension Declaration Form, provided free of charge by the Ministry of Finance.

Along with the form, claimants should provide photocopies of their ID cards and those of the two declarants listed in the form. These copies should be properly certified by the local chief.

Provide Birth Certificates

If the children are undergoing full-time education, claimants must include original letters from the heads of the schools or institutions they attend, signed and stamped by the respective school authorities.

Claimants should obtain a sworn affidavit or a letter on the original letterhead from their local area chief, supporting their relationship with the deceased. This document should bear the original chief’s stamp.

Guardian’s Birth Certificate (if applicable)
If the guardian of the minor children is a child of the deceased pensioner, both the original and a photocopy of the guardian’s birth certificate should be provided.
