Complete the Declaration Form
Claimants must accurately and truthfully fill out the Dependants Pension/Widow’s & Children’s Pension Declaration Form, provided free of charge by the Ministry of Finance.

Include Photocopies of ID Cards
Along with the form, claimants should provide photocopies of their ID cards and those of the two declarants listed in the form. These copies should be properly certified by the local chief.

Provide Birth Certificates
